ADAPTAÇÃO E ESTUDO DE CONFIABILIDADE DA “ENTREVISTA PARA LEVANTAMENTO RETROSPECTIVO DO INÍCIO DA ESQUIZOFRENIA”  - IRAOS -

Nesta última década vários estudos têm se voltado para o  período inicial da psicose, considerando este um período crítico para o prognóstico da doença. Uma das maiores limitações para intervir nesta fase é sua identificação. Os sintomas não são específicos de um quadro psicótico e a avaliação deste período tem que ser retrospectivo. Para suprir estas dificuldades, foram desenvolvidos instrumentos para caracterizar a fase prodrômica e a fase aguda inicial da psicose. A IRAOS (Interview for the Retrospective Assessment of the Onset of Schizophrenia) foi desenvolvida por Häfner et al, sendo uma das primeiras entrevistas construídas com este objetivo e tem sido utilizada em vários centros internacionais.
O objetivo deste estudo é adaptar e testar a confiabilidade da IRAOS  em pacientes com diagnóstico de esquizofrenia. Com este intuito duas pesquisadoras independentes entrevistaram 23 pacientes que preenchiam os critérios diagnósticos do DSM-IV para esquizofrenia, tinham uma duração da doença menor que três anos e que são acompanhados no Programa de Esquizofrenia (PROESQ) da UNIFESP – EPM.
A análise mostrou que os índices de Kappa encontrados na confiabilidade tanto interentrevistadores (entre 0,80 e 1,00) como para a teste - reteste (entre 0,60 e 0,79). foram semelhantes aos encontrados por Häfner et al (1990) por ocasião da elaboração da IRAOS.
A IRAOS mostrou-se abrangente e de fácil aplicação por pessoas habituadas a entrevistas padronizadas. Possivelmente ela será de grande utilidade para novas pesquisas, especialmente as que investigam o período inicial da esquizofrenia.

ABSTRACT
In this last decade many studies have addressed the onset period. One of the greatest difficulties is its initial identification. The symptoms are not specific of a psychotic diagnosis and the evalution of this period must be retrospective. To suppress these difficulties instruments have been developed to characterize the prodromic phase, as well as the acute initial phase of the psychosis. The IRAOS -Interview Retrospective Assessment of the Onset of Schizophrenia  - interview was developed by Häfner et al. (1990), and is one of the first tools made for this purpose, it is now used in many international centers.
The aim of this study is to adapt and test the reliability of the IRAOS in schizophrenic patients. This study involved 23 DSM-IV schizophrenic patients which where under treatment in the Schizophrenia Program of the Universidade Federal de São Paulo / Escola Paulista de Medicina. The results demonstrated that the IRAOS achieved a good realiability between the investigators (Kappa ranger from 0,80 to 1,00) as well as good retest reliability (Kappa ranger from 0,60 to 0,79) These coefficients were similar to the ones found by Häfner et al. (1990).
The interview IRAOS has proven to be of easy application. We believe that the IRAOS can be useful in researches involving subjects in early course of schizophrenia.
